I hereby understand, agree and confirm that:

1. The above details are entered/selected by me and will be updated in my Individual Account(s) and across all above-mentioned products

2. All callbacks or checks to confirm the genuineness of any transaction, as deemed fit by the ICICI Bank will be sent to my updated mobile number

3. All correspondence in relation to the Account should be sent to the communication address as updated in this application

4. I shall indemnify ICICI Bank for any loss/damage/liability that arises due to updation of the communication address provided by me

5. Basis this request, my address details mentioned in the Aadhaar will be updated as my permanent address in the Bank records.

6. ICICI Bank is hereby authorised to use my Aadhaar/Virtual ID details (as applicable) for verification of this application form, for Aadhaar-based electronic signature and share it with Protean e-Governance Technologies Ltd (Protean e-gov) for verification purposes.

7. In the event of any change in KYC details, I/we shall inform and submit updated documents (i.e., address, contact details, profile etc.) to the Bank within 30 days from the date the change was made.

8. I hereby declare that I authorise ICICI Bank to download/fetch my CKYCRR record for the purpose of processing of re-KYC/demog update application and/or updating the CKYCRR record reference number in Bank’s records or any other related purposes.

Aadhaar Consent:

I voluntarily submit my Aadhaar details to the Bank for biometric based e-KYC authentication for establishing my identity and demographic details. The purpose of collecting Aadhaar has been explained to me. Upon authentication, UIDAI may share my demographic information (including photograph), which will be used by the Bank only for the purpose mentioned above or as required by law. I consent to sharing my Aadhaar details with UIDAI, NPCI, regulatory or statutory authorities as required under applicable laws. I have been informed that my biometric information will not be stored by the Bank.
