Aadhaar Consent and Terms & Conditions- Demog Updated

I hereby understand, agree and confirm that:

• The details mentioned above are entered/selected by me and will be updated in my Account

• All SMS alerts/communications related to my Account will be sent to this updated Mobile Number or e-mail ID

• All callbacks or checks to confirm the veracity of any transaction will be sent to this updated Mobile Number or e-mail ID

• ICICI Bank is hereby authorised to use my Aadhaar/Virtual ID details (as applicable) for verification of this application form, for Aadhaar-based electronic signature and share it with Protean e-Governance Technologies Ltd. (Protean e-gov) for verification purposes

• ICICI Bank is hereby authorised to verify my Permanent Account Number (‘PAN’) details with Protean e-gov

• In the event of any change in KYC details (i.e., address, contact details, profile, etc.), I/we shall inform and submit updated documents to the Bank within <30> days from the date the change was made. Any modification can be communicated to ICICI Bank through the Bank’s available channels.

Aadhaar Consent

I have been provided various options by ICICI Bank for establishing my identity for the purpose of PAN/e-mail/Mobile number update in my account with ICICI Bank and I voluntarily submit my Aadhaar details to the Bank for biometric based e-KYC authentication:

• to establish my identity and demographic details. The purpose of collecting Aadhaar has been explained to me.

• Upon authentication, UIDAI may share my demographic information (including photograph), which will be used by the Bank only for the purpose mentioned above or as required by law. I consent for sharing my Aadhaar details with UIDAI, NPCI, regulatory or statutory authorities as required under applicable laws.

• I have been informed that the Bank will not store my biometric information.

I have read and understood the declarations, consents and terms governing this application form/request and hereby accept the same.
